QR codes fail for boring, physical reasons far more often than technical ones — printed too small, placed where a phone cannot focus, or pointed at a URL that changed after the labels were printed.
A QR code needs roughly two centimetres square to scan reliably at arm's length, and more if it will be read from further away or in poor light. Keep a clear margin around all four sides — a code printed hard against a border or over a busy background will scan inconsistently, which in practice means people stop trying.
Encode a URL on your own domain rather than a third-party short link. Short-link services disappear, change owners, and start charging. A code printed on a thousand labels outlives whichever provider seemed convenient the day it was made, and the URL inside it cannot be edited once it is on a wall.
A bare code on a shelf gets ignored. A line of text beneath it — 'scan to report a fault', 'scan for the manual' — is the difference between a code that gets used and one that becomes visual noise.
